Here are a few things to know before setting up your first therapy appointment.
Initial Phone Contact
I offer an optional, free 10–15 minute phone consultation. We will briefly discuss the reasons you wish to begin therapy and have an opportunity to connect prior to setting up your in-person appointment. Please call or email to arrange a time.
Consultation Period
Your first two to three sessions are designed as a time for me to get to know your present concerns and history. This time is also your opportunity to decide whether you feel confident that I am the best person to help you.
Payment and Insurance
I am an out-of-network therapist. Most insurance companies within the Washington, DC area will reimburse a percentage of mental health costs for out-of-network providers. I bill at the time of service, and I accept cash and personal checks as payment. Each month, you will receive a statement detailing the services provided, the amount paid, and the billing codes needed for you to receive entitled benefits from your insurer.
Before Your First Visit: Patient Forms
New patients, you may print and fill out the forms below and bring them to your first session or email to Dr. Snyder. Please also be sure to read the Therapist-Patient Services Agreement and DC Notice Form. If you prefer, you may plan to arrive 10 minutes early to your first session to complete them.
NEW PATIENT FORMS:
Authorization for Release of Information Form* (Required if communication with other doctors or family members is needed to coordinate care or billing.)
*Forms can be digitally filled out and signed with a free PDF reader (e.g., Adobe Acrobat Reader, Lumin PDF) or printed and completed by hand. Once you’ve completed them, please email to drangelamsnyder@gmail.com.
